  The entries are suggestions and should be adapted to your setting.
Example: Political Secret: The true heir to the throne is living under a false name as a local blacksmith's apprentice. - This could instead be the daughter of the major working as a maid or the son of space faction's leader working as a cook on a pirate spaceship.

3 Political Secret Hidden corruption, an unholy alliance, or a secret heir affecting local leadership. The current governor is a puppet being blackmailed by a notorious criminal syndicate. The local noble is secretly funding the very bandit groups they publicly vow to destroy. The true heir to the throne is living under a false name as a local blacksmith's apprentice.
4 Economic Reality A sudden shortage, a hidden trade route, or a crisis involving currency and resources. Nearly half of the gold coins circulating in the market are highly convincing lead counterfeits. The local mine actually dried up months ago, and the owners are hiding it to avoid panic. A secret underground tunnel bypasses the city gates to smuggle untaxed luxury goods.
5 Geographical Feature A strange cave system, a moving landmark, or a hidden environmental hazard. The nearby river completely reverses its flow for one hour during every full moon. The dense woods shift locations slightly at night, making standard maps unreliable. A massive, uncharted sinkhole in the hills has started venting warm, glowing purple gas.

The temple basement contains a sealed altar dedicated to a primordial god of destruction. The high priest has noticed the holy statues have begun crying black tears at midnight.
8 Monster Behavior An unknown weakness, a strange migration pattern, or an unusual diet of local beasts. The predatory monsters in the hills will completely ignore anyone carrying fresh mint. The local wolves are migrating away because something much larger has claimed the forest. The territorial monsters are actively attracted to the sound of ringing church bells.
9 Architectural Secret A hidden room, a structural flaw, or a builder's curse hidden inside a major building. The local manor house has a hidden panic room accessible only through a fireplace. The city's main defensive wall has a severe structural flaw that a single blast could breach. The architect cursed the castle so that no door can ever be completely locked at night.
10 Recent Crime A high-profile theft, a mysterious disappearance, or a major cover-up by authorities. The local banker didn't get robbed; they stole the money themselves and blamed bandits. A prominent scholar vanished from their locked study, leaving only a puddle of water. The guard covered up a murder by framing a local beggar who has already been executed.

A massive cache of weapons and armor is buried beneath the floor of the local granary. The local garrison is planning a surprise midnight raid on a nearby peaceful settlement.
14 Ancient Myth A local bedtime story, folk song, or legend that turns out to be entirely, literally true. A sleeping giant forms the mountain range and awakens if the silver valley bell is rung. 15 Family Feud A quiet, bitter shadow war between two powerful local bloodlines or factions. Two leading farming families are poisoning crops to drive rivals to bankruptcy. The Mayor and High Priest are rivals, leading guards to arrest acolytes on fake charges. Two merchant guilds hire mercenaries to ambush trade caravans, blaming fake bandits.
16 Environmental Hazard Poisonous spores, unstable ground, or a creeping natural threat to the area. The local swamp has pockets of odorless gas that ignite instantly near open flames. The cliffside trail is highly unstable and will collapse under the weight of horses. A parasitic vine is spreading rapidly, strangling the roots of all local food crops.
17 Foreign Influence A spy network, an exotic illegal trade, or alien cultural imports changing the town. The local tavern is a front for foreign spies intercepting mail meant for the king. A noble's carriage carrying a tax fortune sank into the bog during an old chase. The local church altar hides a secret compartment containing a powerful holy relic.
19 Underground Group A hidden thieves' guild, a rebel cell, or a forbidden cult operating right under people's noses. The local orphanage is actually a secret training ground for an elite guild of thieves. An underground anti-monarchy rebel cell meets in the crypts beneath the cemetery. A cult worshipping an elder sea god meets in the sea caves during low tide.
20 Imminent Danger A broken structure, an awakening beast, or an approaching raid threatening immediate harm. The upstream dam has developed severe cracks and will burst during the next heavy rain. A long-dormant volcano nearby has begun trembling, venting smoke, and ash. An aggressive horde of raiders has been spotted a half-day's march from the borders.
