The Grakh'tor withdrew in the autumn. In the spring following, the fever came upon their western clans. It did not come upon us. Their warriors who returned to their homes returned to fires that could not warm them. Their people call it Vor'grak'vel and treat it as a matter for their shamans. We have logged the observation in these records because we log observations. We have no opinion on their theology.

The Vor'grak'vel, rendered in Roman scholarly Latin as Pestis Poenitentiae and known within the confederacy as the Penance-Sickness, was the epidemic that swept the Grakh'tor Clan Confederacy within two seasons of the Keth'dural Grak's conclusion, killing more Grakh'tor than the war itself had over its full forty years. The pathogen was carried back to the western territories by warriors returning from the Iron Spine campaign, spread with catastrophic speed through the returning warrior population and outward into the general clan populations, and by the epidemic's close approximately three years after its emergence had reduced the confederacy's total population by approximately eighteen percent.

The Vor'grak'vel is the founding event of the Vor'keth order's public health authority and one of the most consequential single events in Grakh'tor institutional history. The order's response to the epidemic, which included the declaration of theological framing (the sickness as war-gods' penance for the confederacy's failure to achieve territorial victory in the Keth'dural Grak), the establishment of the agricultural zone access protocols that remain in modified form to the present day, and the systematic recording of the epidemic's dead in the Grakh'shen Dal at Keth'voral by clan and named individual, transformed a public health catastrophe into a foundation of Vor'keth institutional authority that the confederacy has deferred to across the thirteen centuries since. The epidemic has not recurred. The mechanism by which the pathogen ceased circulating in the Grakh'tor population is not documented in any source available to this author, and Plinius considers it, on balance, an open question of some current relevance.

The Vor'keth's private conclusion, preserved in the inner council's records at Keth'voral and not transmitted to any secular authority, was that the pathogen had emerged from a population that surface Grakh'tor scholarship of the period had not yet formally identified: the goblin communities operating in the outer margins of the dwarven mining network by the middle-war period, whose environment had incubated a pathogen that the Grakh'tor immune profile could not accommodate but the dwarven and goblin immune profiles could. This conclusion has been held by the Vor'keth inner council for thirteen centuries. It has not been shared with the confederacy at large, with the Khazadum, or with any Roman scholar. It is one of the Vor'keth's most carefully guarded pieces of institutional knowledge, and its disclosure would substantially alter the current understanding of goblin political history.

Transmission & Vectors

The pathogen was transmitted primarily through respiratory contact between infected and uninfected individuals, with a secondary transmission pattern through shared water sources and shared eating vessels. Incubation between exposure and symptom onset was approximately eight to twelve days by the surviving retrospective accounts, and the infected individual became transmissible approximately three days before the appearance of external symptoms, which is one of the epidemic's most consequential characteristics and the mechanism by which the pathogen spread with such catastrophic speed through the returning warrior population. Warriors returning to their clan settlements were transmissible for approximately three days before anyone in the clan had reason to suspect that anything was wrong.

Cross-species transmission has not been documented. The dwarven side of the western foothill front produced no Vor'grak'vel cases either during the war or after its conclusion, despite forty years of continuous close contact between dwarven defenders and orc attackers at Kharak-Bel Hold and the secondary foothill positions. The Roman scholarly view, established through the post-Foedus Khazadum medical correspondence, has treated this species-specificity as evidence that the pathogen was fundamentally species-specialised to the Grakh'tor and could not have crossed to any other people; Plinius considers this conclusion partially correct in its practical implication but incomplete in its explanatory framing, and notes that species-specificity in a pathogen is more often a function of specific immunological factors than of any inherent species barrier.

The pathogen was in fact transmissible to any species with a comparable respiratory system, but the goblin population that harboured it endemically had, over generations of exposure, developed the specific immunological adaptation that permitted asymptomatic carrier status. The dwarven population's absence of cases resulted from the specific segregation of dwarven and goblin working areas at the mining chain, which limited direct dwarven exposure to a level insufficient for infection to establish, not from any species-inherent immunity. If a modern equivalent were to emerge from the same or a similar source, the dwarven population would be at genuine risk of infection, contrary to what the historical record has been read to suggest. The Vor'keth's inner-council understanding of this point has never been communicated to Roman medical scholarship. The Vor'keth are aware that their private understanding of transmission is not the understanding that Roman and Khazadum public health frameworks operate on.

Causes

Roman medical scholarship, which has treated the Vor'grak'vel primarily as a comparative case in discussions of the Pestis Cinerea (500 A.P.) rather than as a subject of independent investigation, has generally attributed the epidemic's cause to a novel pathogen encountered during the Iron Spine campaign, brought back to the western territories by returning warriors, and unmatched by any prior Grakh'tor immunological exposure. The specific pathogen and its natural reservoir have not been identified in any Roman source. Roman scholarly writing tends to attribute causation to the campaign environment of the Iron Spine's western foothills without further specificity, which reflects the limits of Roman investigative access to the region rather than any settled conclusion about the pathogen's origin.

The Grakh'tor cultural understanding of the epidemic's cause is theological rather than epidemiological. The Vor'keth's public declaration, made within weeks of the epidemic's initial spread and codified in the Grakh'shen Dal's contemporary record, framed the sickness as Vor'grak'vel: literally, the Wound-Penance, or in current translation the Penance-Sickness, a divine imposition by the war-gods in acknowledgement of the confederacy's failure to achieve territorial victory in the Keth'dural Grak. This framing has been the confederacy's operative understanding of the epidemic's cause for thirteen centuries. It has been consistently maintained in all Vor'keth teaching, in all secular Grakh'tor commemorative practice, and in Uzrul Ironteeth's own account when Plinius has raised the question.

The order's inner council concluded, within approximately six months of the epidemic's emergence, that the pathogen was probably of goblin-population origin (see Transmission DM notes above that the epidemic could not be halted through any available intervention beyond isolation and hygiene protocols; and that public communication of the natural etiology would produce social and political consequences (loss of confidence in the returning warrior population, potential inter-clan violence, and the collapse of the confederacy's still-recent unification) that the confederacy could not survive concurrent with the epidemic itself. The theological framing was chosen because it converted a public health crisis into a spiritual event that the order could administer, gave meaning to warrior deaths that the natural etiology could not, and established the specific institutional authority that has served the confederacy across the thirteen centuries since. The framing was not a mistake. It was a decision, and it worked. Whether it was the right decision remains a question that the order has not, to Plinius's knowledge, ever revisited in its public teaching.

Symptoms

The epidemic's symptomatic progression followed a consistent pattern across the surviving retrospective accounts. Initial symptoms, appearing approximately eight to twelve days after exposure, comprised sustained high fever (in Grakh'tor terminology 'the fire that will not warm,' from the specific paradox of a burning fever accompanied by an intense sensation of cold), pronounced fatigue disproportionate to physical activity, and a specific pattern of muscle ache concentrated in the limbs and lower back that Uzrul Ironteeth's family tradition (his great-great-grandfather survived the epidemic in the early middle phase and left an oral account) describes as 'the ache that reminds a warrior of an old wound they had forgotten they carried.'

The second-stage progression, developing over the following three to five days, comprised the appearance of dark red-purple skin discolouration in patches across the torso and neck (in Grakh'tor terminology 'the war-marks the war-gods have added,' a specific theological framing preserved in the Grakh'shen Dal's contemporary record), respiratory difficulty progressing to sustained cough, and a distinctive alteration in the affected individual's voice that made speech laborious and produced a rasping quality that the surviving accounts consistently identify as the epidemic's most recognisable single symptom. The Grakh'tor cultural memory retains the specific phrase 'to speak with the rasp' as an idiom for the moment at which the epidemic became visible in a specific individual, and the phrase remains in current Grakh'tor usage as a general metaphor for the moment at which a serious problem becomes undeniable.

The third-stage progression, developing in cases that would prove fatal, comprised progressive respiratory failure over three to seven days culminating in death by pulmonary collapse. Cases that resolved rather than progressed to fatal outcome did so through a specific and dramatic fever break at approximately day fourteen to eighteen from initial symptom onset, followed by a prolonged recovery period of six to twelve weeks characterised by residual fatigue, respiratory weakness, and the persistence of the distinctive rasping voice quality for months or occasionally permanently. Approximately forty percent of infected individuals survived; approximately sixty percent did not.

Treatment

No effective treatment was available to the Grakh'tor of the epidemic period, and no effective treatment has been developed in any medical tradition since. The Vor'keth's contemporary response comprised isolation of confirmed cases in dedicated care compounds established rapidly across the confederacy during the epidemic's early weeks, systematic hygiene protocols (boiling of water, separation of eating vessels, cremation of the dead) that the order administered directly and enforced through its ceremonial authority, and the provision of palliative care to the dying warriors specifically framed within the theological interpretation of the epidemic. The dying warriors were told they were paying a penance the war-gods had imposed. This framing produced, across the surviving accounts, a specific quality of end-of-life peace that the natural etiology could not have provided.

Roman medical scholarship's comparative discussions of the Vor'grak'vel in the context of the Pestis Cinerea have consistently concluded that the palliative care approach, in the absence of any effective curative intervention, was the most that either people's medical tradition could have accomplished. Plinius agrees with this conclusion in its practical implication. He notes with some professional interest that the Vor'keth's decision to unify palliative care with theological interpretation produced psychological and social outcomes that Roman medicine has generally been less capable of achieving in comparable pandemic situations, and considers this one of the more instructive cross-cultural comparisons available in the primary continent's medical history.

No treatment addressed the pathogen itself in the epidemic period. The Vor'keth's inner council has, however, quietly maintained across the thirteen centuries since a specific research programme on the pathogen's characteristics, using the preserved records of the epidemic's cases and the small number of preserved specimen samples that the order has retained under conditions Plinius has been unable to characterise. The programme has produced, in the current century, a preventive protocol that the order believes would be effective against a recurrence of the same or similar pathogen if it were deployed early in an outbreak's development. The protocol has not been publicly disclosed, has not been trained into the general shamanic population, and remains in the inner council's exclusive knowledge. Whether the protocol would be released in the event of a recurrence is a question the DM should determine. The order's institutional preference is for controlled release; if the recurrence coincided with Rift XIII cosmological pressure, the order might well determine that its authority is better served by permitting the theological framing to reassert without the natural intervention that the protocol would enable

Prognosis

Historical prognosis for Vor'grak'vel infection was approximately forty percent survival, sixty percent mortality, with the specific determinant of outcome being the fever break at day fourteen to eighteen from symptom onset. Individuals who survived to the fever break generally survived the illness overall, though with the prolonged recovery period described above; individuals whose fever did not break by day eighteen almost invariably progressed to fatal respiratory failure within the following seven days. The specific factors that determined which individuals experienced the fever break and which did not have not been established by any medical tradition since. Roman comparative discussion has generally attributed the variation to individual constitutional strength, which is the register Roman medicine uses when it has no better explanation available; Plinius considers this attribution unsatisfying but has not been able to propose a better one.

Prognosis for the epidemic as a whole, at the confederacy level, was that the pathogen would exhaust its susceptible population within approximately three years and cease to circulate. This is what happened. The mechanism by which the pathogen ceased circulating in the Grakh'tor population after -852 A.P. is not documented in any source available to Plinius. It is possible that the surviving population had developed sufficient immunity to prevent further transmission; it is possible that the pathogen mutated to a form that no longer supported human infection; it is possible that the Vor'keth's isolation and hygiene protocols achieved a level of transmission suppression that reached the threshold below which sustained circulation could not continue. Each of these mechanisms is consistent with what is known. None has been established.

The pathogen ceased circulating in the Grakh'tor population after -852 A.P. through a combination of surviving-population immunity and the Vor'keth's isolation protocols, but it did not cease to exist. The goblin communities that had originally hosted the pathogen continued to host it, and continue to do so as of 1200 A.P. The Vor'keth are aware of this. They have monitored, through the border markets and through the frontier clan networks, for signs that the pathogen has re-crossed to the Grakh'tor population in the fourteen centuries since the epidemic. It has not. Whether it could, whether the current pre-Rift XIII cosmological pressure could produce conditions that facilitate re-crossing, and whether the Vor'keth's preventive protocol would be released in time to prevent a second catastrophic epidemic, are questions the DM should determine. If a recurrence emerges as a campaign development, the goblin population's continued asymptomatic carrier status would be one of the more consequential political revelations the campaign could produce.

Sequela

Survivors of the epidemic frequently retained residual effects for the remainder of their lives. The most common and best documented sequela was persistent respiratory weakness, which reduced the survivors' capacity for sustained physical exertion and shortened their working lives by an estimated ten to fifteen years relative to the pre-epidemic Grakh'tor life expectancy. The distinctive rasping voice quality persisted permanently in approximately one third of survivors and served, across the following generation, as the audible marker of Vor'grak'vel survival: any Grakh'tor of that generation whose voice bore the rasp had survived the epidemic, and was accorded a specific cultural respect that was codified in the Vor'keth's post-epidemic institutional teaching as the doubly-acknowledged status. Approximately twenty percent of survivors also developed a chronic cardiac weakness that produced elevated mortality across the following decade; this pattern was documented in the Grakh'shen Dal but did not enter the confederacy's general cultural memory.

The confederacy-level sequela extended for approximately two generations beyond the epidemic's close and included substantially altered clan power dynamics (clans that had lost disproportionate numbers of warriors and elders emerged from the epidemic with reduced political weight), a shift in the confederacy's demographic composition toward the frontier clans (whose warrior population had contributed the largest proportion of returning-warrior deaths) that took approximately three generations to normalise, and the specific institutional entrenchment of the Vor'keth's public health authority that persists in modified form to the present day. The confederacy that had entered the Keth'dural Grak was not the confederacy that emerged from the Vor'grak'vel. The transition from one to the other was mediated by the epidemic more than by the war.

Affected Groups

The epidemic affected the entire Grakh'tor population without geographic or clan exception, but the initial and most severely affected group was the returning warrior population of the western foothill campaign. Approximately eighty-five percent of returning warriors contracted the disease within the first six months of the epidemic's spread; of these, approximately sixty percent died. The returning warrior mortality was concentrated in the ~-855 to ~-854 A.P. window, when the pathogen was spreading through this initial highly-exposed population and had not yet reached the general clan populations at scale.

The secondary spread, from returning warriors to their home clans, produced the epidemic's larger absolute mortality across the following two years. Elders, young children, and pregnant women showed higher mortality within the general clan populations, following the general pattern that epidemic viral pathogens produce in populations with no prior exposure. The frontier clans whose returning warriors had provided the largest campaign contingents suffered the highest total mortality; the more distant western coast clans suffered the lowest total mortality, both because their warrior contingents had been smaller and because their geographic isolation permitted the Vor'keth's transmission protocols to work more effectively. Roman comparative discussion has consistently noted the epidemic's geographic pattern as evidence that transmission protocols can meaningfully affect pandemic outcomes given sufficient institutional authority to enforce them; Plinius considers this observation correct and notes it in his own writing on the Pestis Cinerea protocols.

The one susceptibility pattern that the Vor'keth's contemporary investigation established but never publicly documented was the substantially reduced infection rate among Grakh'tor of clans with historical intermarriage into the western coast populations. The specific genealogical detail is preserved in the Grakh'shen Dal but has never been transmitted to outside scholarship. The pattern is consistent with the pathogen's goblin-origin hypothesis (the western coast clans had had longer historical exposure to the goblin population and had developed partial immunity), and would represent, if released, further confirmation of the goblin-origin theory. The Vor'keth have not released it. Uzrul is Krul'dent Dur; his clan is among those with reduced historical exposure, and he has not commented on any family tradition regarding the epidemic's clan pattern.

Hosts & Carriers

No animal host or vector has been documented in the Roman scholarly literature. The pathogen appears to have transmitted directly between infected humanoid hosts without intermediate animal reservoir, at least within the epidemic's active period in the Grakh'tor population. The Roman comparative discussion has consistently noted this as one of the epidemic's distinguishing characteristics relative to other primary continent pandemic events.

DM ONLY
The goblin population is the pathogen's endemic host and continues to carry the pathogen asymptomatically as of 1200 A.P. This is the single most consequential piece of undocumented biology in the Vor'grak'vel's history. The Vor'keth's inner council knows. Nobody else does. If the goblin diplomatic negotiations produce a moment at which the Grakh'tor confederacy and the Zrek'vali warrens establish sustained direct contact of the kind that Skrix Vreth's negotiations may enable, the epidemiological consequences would need to be specifically managed by parties who currently have no framework for even discussing the question. The Vor'keth's inner-council awareness of the risk is one of several factors driving the order's specific caution about the current negotiations. Vezra Quickfingers, from the goblin side, is aware of the pathogen's existence but has not connected it to the historical Grakh'tor epidemic; goblin institutional memory of that period is limited by the Guild-suppression pattern documented in the Khar-Vulm article.

Prevention

No effective preventive measure was available to the Grakh'tor of the epidemic period beyond the isolation and hygiene protocols the Vor'keth administered during the epidemic itself. The confederacy's response to the epidemic did not include the development of any prophylactic practice against future recurrence, because no future recurrence was anticipated in the theological framing the Vor'keth had established: the epidemic was penance for a specific war, and no comparable penance was expected to be imposed in the absence of a comparable failure. The confederacy has therefore proceeded across the thirteen centuries since without a specific preventive practice against Vor'grak'vel.

Roman scholarly discussion has, in the medical literature since the Pestis Cinerea, occasionally proposed that comparative study of the Vor'grak'vel and Pestis Cinerea protocols might yield insights applicable to future pandemic response. The proposals have not been acted upon at any institutional level, primarily because the specific Vor'keth protocols have not been made available to Roman medical scholarship in sufficient detail to support the comparative work.

The Vor'keth's preventive protocol, described under Treatment DM notes above, exists and could be deployed. Whether it would be deployed against a recurrence in the current pre-Rift XIII window depends on institutional decisions that the inner council has not committed to any documented record. The specific factors the council would weigh include: the severity of the emerging outbreak, the political situation at the moment of emergence (a stable confederacy under a confident Vor'grak might see the protocol released; a fractured succession situation might not), and the specific character of the cosmological pressure at the time. If Rift XIII produces conditions that facilitate re-crossing of the pathogen, and if the succession situation at that moment is unresolved, the Vor'keth might well conclude that theological reframing serves the confederacy better than pharmacological intervention, and permit the protocol to remain unreleased. This decision, if it were made, would be one of the more consequential institutional choices the campaign could produce.

Epidemiology

The epidemic's spread through the Grakh'tor Confederacy in the ~-855 to ~-852 A.P. window followed a specific and now well-documented pattern. Initial cases emerged in the eastern frontier clans that had provided the largest returning warrior contingents, at settlements located within approximately a hundred kilometres of the eastern border with the Iron Spine foothills. The pathogen spread outward from these initial sites at a rate of approximately three to four hundred kilometres per season, following the confederacy's principal internal communication routes and reaching the far western coast populations within approximately eighteen months of the initial emergence. Peak weekly mortality occurred approximately twelve months after initial emergence, in the summer of ~-854 A.P., with approximately two thousand deaths per week across the confederacy at peak.

The epidemic's decline was gradual across the following eighteen months, with weekly mortality falling to approximately three hundred by early ~-852 A.P. and to essentially zero by the autumn of that year. The final documented case in the Grakh'shen Dal is recorded in the eighth month of ~-852 A.P.; no subsequent cases have been documented in any source available to Plinius. Total confederacy mortality across the three-year epidemic is estimated at approximately eighteen percent of the pre-epidemic population, or approximately 800,000 individuals from a pre-epidemic confederacy population of approximately 4.5 million. The comparison to the Keth'dural Grak's approximately 120,000 confederacy dead across forty years is one of the more sobering pieces of arithmetic in primary continent pandemic history.

The epidemic's specific geographic pattern maps closely onto the returning warrior population's clan-of-origin distribution across the confederacy, which is what the surface record suggests. Beneath this pattern, however, is a subtler geographic pattern that the Vor'keth's inner council has documented and that would repay direct investigation: the mortality rate showed statistically significant variation across clans with historical intermarriage into the western coast populations (see Affected Groups DM notes), and the specific mortality-by-clan data preserved in the Grakh'shen Dal supports the goblin-origin hypothesis in a way that the surface record does not. A player character with sufficient access to the Grakh'shen Dal to review the mortality-by-clan data could reconstruct the pattern and reach the same conclusion the inner council reached thirteen centuries ago. Whether this reconstruction would be permitted by the order, and what the order would do if it discovered that an outside party had performed the analysis, are open questions the DM should determine.

History

The Vor'grak'vel emerged in the season immediately following the Keth'dural Grak's disengagement (~-855 A.P.), spread through the confederacy across the following three years, and ceased circulating in the general population by ~-852 A.P. The epidemic's contemporaneous documentation in the Grakh'shen Dal is the most detailed surviving record of the event; the Kharak-Bel Chronicle provides parallel documentation from the dwarven side, primarily noting the epidemic's species-specificity to the Grakh'tor and the dwarves' absence of cases. Roman scholarship encountered the Vor'grak'vel through post-Foedus Khazadum medical correspondence in the third and fourth centuries A.P., and the epidemic has been discussed comparatively in Roman medical literature since the Pestis Cinerea (500 A.P.) established the primary continent's first documented Roman-era pandemic.

The most detailed Roman scholarly treatment of the Vor'grak'vel prior to this article is the comparative discussion in Antonius Marcellus's De Pestilentiis Aethermarchensibus (876 A.P.), which draws on Bera Khar-Thul's grandfather's translation of relevant Kharak-Bel Chronicle passages and provides the framework for subsequent Roman comparative work. Antonius's treatment is scholarly and generally reliable within its acknowledged limits, but has not been updated since; the current article represents the first substantial Roman scholarly treatment of the epidemic since Antonius's work more than three centuries ago.

Antonius Marcellus, in his 876 A.P. treatment, was permitted access to portions of the Grakh'shen Dal's mortality-by-clan data that no subsequent Roman scholar has been permitted access to. He noticed the pattern of reduced mortality in clans with western coast intermarriage. He did not publish this observation. His personal notes, preserved in the Imperial Academy's classified frontier medical section, contain the observation with a specific note that its publication would 'exceed the scope of the current work and require inquiry beyond what the present author is positioned to undertake.' Antonius appears to have understood that he had encountered a piece of evidence pointing toward a specific hypothesis about the epidemic's origin, and to have chosen not to pursue it publicly. Plinius has read Antonius's notes. He has not, to date, decided whether to include Antonius's suppressed observation in his own published treatment. The current article's decision has been to reference the pattern in the DM Affected Groups section without publishing the underlying hypothesis. The DM should determine whether Plinius revisits this decision as the campaign progresses.

Cultural Reception

Grakh'tor cultural memory of the Vor'grak'vel is elaborate, specific, and structured around the theological framing the Vor'keth established during the epidemic itself. The epidemic's dead are remembered not as victims of biological catastrophe but as participants in a penance the confederacy paid collectively, and the returning warriors who died of the sickness after surviving the war are commemorated in the Grakh'shen Dal alongside those who died in the war itself, under the specific category of warriors who paid the full price. The doubly-acknowledged status conferred on survivors of both war and sickness remains a specific piece of confederacy cultural memory, referenced in current Grakh'tor cultural practice at Keth'voral dedications and in the standard opening of moot proceedings when the deep historical record is invoked.

The Grakh'tor cultural response to survivors of the epidemic during the two generations following was one of specific respect and accommodation.
